New patients: what to bring
A little preparation helps us give you our full attention from the moment you arrive. Please plan to arrive about 15 minutes early to complete any remaining paperwork and verify your information.
Your visit checklist
- A valid photo ID

Insurance & billing
We work with most major plans and are glad to help you understand your coverage before your visit.
Hartwell Health accepts most major commercial insurance plans and Medicare. Because plans and benefits vary, we encourage you to verify your coverage directly with your insurer and with our office before your appointment — including whether a referral or prior authorization is required for a cardiology visit or diagnostic study.
When a service requires prior authorization, our team will help coordinate it with your plan and your referring physician. We will do our best to keep you informed about what is covered and what to expect, but final coverage and cost-sharing are determined by your insurer.

For patients

Do I need a referral to be seen?
Not necessarily. We welcome self-referrals as well as referrals from your primary-care physician. That said, some insurance plans require a referral or prior authorization before a specialist visit, so we recommend checking with your plan. Our office is happy to help confirm what your coverage requires.
How should I prepare for a stress test?
Wear comfortable clothing and walking shoes, and plan to avoid caffeine for about 24 hours beforehand. Ask us whether to hold any medications before your test, and let us know if you have diabetes so we can advise on eating and medication timing. We will give you specific instructions when your study is scheduled.
When will I get my test results?
Most cardiac studies performed in our office are reviewed by your cardiologist within a few business days. We will contact you with your results and any next steps, and they will also be shared with your referring or primary-care physician. If you have not heard from us, please call the office.
How do I request my medical records?
Call our main line at (617) 555-0140 or email info@hartwell-health.com to request a copy of your records. We will provide a simple authorization form to complete, and records are typically released within the timeframe required by law. See our Notice of Privacy Practices for details on your rights.
How do I refill a cardiac medication?
The fastest way to refill is to ask your pharmacy to send a refill request to our office. You may also call us at (617) 555-0140. Please allow up to two business days for refill requests, and reach out before you run out so there is no gap in your medication.
